1. अनुलग्नक-ए: पासपोर्ट और निरक्षर व्यक्ति के लिए उनकी जन्म तिथि और शिक्षा योग्यता साबित करने के लिए पहचान प्रमाण पत्र।

2. अनुलग्नक-सी: नाबालिग पासपोर्ट आवेदन के लिए एक या केवल माता-पिता।
