Immediate Action for Lost Passport

File a Police Report (FIR):
This is mandatory. You need a copy of the First Information Report (FIR) to prove the passport was lost or stolen and to prevent potential misuse.

2. Apply Online for a “Re-issue.”

Portal: Go to the official Passport Seva Website.
“Apply for a Fresh Passport/Re-issue of Passport.”
Reason for “Loss/Theft of Passport.”
Complete & Pay: Fill out the rest of the form carefully with your correct details. Pay the applicable fee online and schedule an appointment

Lost Passport Documents

II	FIR or G.D number from the Police Station
III	Application Form: A printout of the online application form, a submission, and an appointment letter.
IV	Birth Certificate: issued by a Municipal Authority or District Office of the Registrar of Births & Deaths. After 26/01/1989, a birth certificate is compulsory; otherwise, a birth certificate is not needed.
V	Identity Proof: ( Two Documents) PAN Card Voter ID Aadhar Card Driving Licence Government ID
VI	Address Proof: (One Document) Aadhaar Card/E-Aadhaar Voter ID (EPIC) issued by the Election Commission. Valid Driving License copy Utility Bills: Electricity bill, Water bill, Gas connection bill, Landline telephone bill Rent Agreement: along with a No Objection Certificate (NOC) from the landlord and the landlord’s proof of address/ownership. Government Bank Passbook with photo and stamp, or pension book.
VII	Qualification: ( One Document) 8th Pass Certificate, Class 10th Admit Card and Mark Sheet Graduation Degree Certificate
VII	Annexure -F, notarized by the court.
IX	All documents must be original with a xerox. Also passport office takes your photo.
