Required Documents Checklist

You find a lost passport with the following documents. 

1. Old Passport Xerox Copy
2. FIR or G.D number from the Police Station
3. Application Form: An online application form and an appointment letter.
4. Birth Certificate:  issued by a Municipal Authority or District Office of the Registrar of Births & Deaths. After 26/01/1989, a birth certificate is compulsory; otherwise, a birth certificate is not needed.

  • Government Bank Passbook with photo and stamp, or pension book.

How to Apply for a Lost Passport in India

File an FIR or GD

1. If you are in India, go to the nearest police station and file an FIR or general diary.
