How to Fill up “Annexure D” for a Minor’s Passport

Annexure D is a service for minor passport applications with a joint declaration from both parents. This annexure is to be filled up only on a computer; it is not to be used manually—list of details as below.

1	Minor Child name and Date of Birth
2	Father and Mother Information, like Name, Address, Contact No, Email, Passport No.
3	Passport Application vide ARN Number
4	Finally, signatures of both parents with ID numbers like PAN, Aadhar, Voter, or Passport.

Minor or Child Passport Documents

i	Application Form: A printout of the online application form, a submission, and an appointment letter.
ii	Birth Certificate (compulsory):  issued by a Municipal Authority or District Office of the Registrar of Births & Deaths
iii	Parents’ Proof: ( Two Documents) Voter ID Aadhar Card Passport
iv	Address Proof: (One Document) Aadhaar Card/E-Aadhaar Passport Address Page of Parents
v	Qualification: ( One Document) School ID card or Mark Sheet 8th Pass Certificate Class 10th Admit Card and Mark Sheet
vi	Annexure -C for Single Parent or Annexure -D for Both Parents.
vii	Photos: The Applicant’s age up to 5 photos need 5 copies. The photo size is (4.5 X 3.5 cm).
viii	All documents must be original with a xerox.
