Annexure “F,” Affidavit—Lost or Damaged Passport (DOWNLOAD)

 

You can find an Annexure ‘F’ or Affidavit, only needed for lost passports or damaged passports as a normal passport application in the Passport Law of India. Also, this process cannot apply to the Tatkal Passport Application in Word format, PDF format, or text format.

 

How to Fillup “Annexure F” for a Lost Passport

Annexure F is used for applications for lost or damaged passports with a single declaration. This annexure is to be completed only on a computer; it should not be used manually—please refer to the list of details below.

Affidavit

I   _____________,  Son  Of ____________, Residing At As ________________________________, Solemnly  Affirm As Follows :-

  1. That I Have Lost My Passport From Travelling In A Bas On ____________ From My Custody And Subsequently I Lodged A General Diary At The ______ Police Station Being  G D S No. _______ Dated _______ And My Passport Number – _______. I have never Lost Any Passport From My Custody Or Otherwise.
  1. That I Have Not Travelled In Any Place On The Lost Passport.
  1. I Have Not Availed Any Tr Concessions / Fts Allowance From Any Authority.
  1. That I Am An Indian National And I Am Not A Non-Resident Indian.
  2. That There Is /Was Any Objection By The Pia In Respect Of The Said Passport.
  3. That I Have Never Deported At Any Time At The Expenses Of The  Government. I  Further Affirm That I Will Take Utmost Care Of My Passport If Issued And The Government Will Be At Liberty To Take Any Legal Action Under The Passports Act, 1967, If The Lapse Is Repeated.

Lost or Damaged Passport Documents

I Old Passport Xerox Copy
II FIR or G.D number from the Police Station
III Application Form: A printout of the online application form, a submission, and an appointment letter.
IV Birth Certificate:  issued by a Municipal Authority or District Office of the Registrar of Births & Deaths. After 26/01/1989, a birth certificate is compulsory; otherwise, a birth certificate is not needed.

  • PAN Card
  • Voter ID
  • Aadhar Card

Address Proof: (One Document)

  • Aadhaar Card/E-Aadhaar
  • Voter ID (EPIC) issued by the Election Commission.
  • Valid Driving License copy
  • Utility Bills: Electricity bill, Water bill, Gas connection bill, Landline telephone bill
  • Rent Agreement: along with a No Objection Certificate (NOC) from the landlord and the landlord’s proof of address/ownership.
  • Government Bank Passbook with photo and stamp, or pension book.

  • 8th Pass Certificate,
  • Class 10th Admit Card and Mark Sheet
  • Graduation Degree Certificate
VII Annexure -F, notarized by the court.
IX All documents must be original with a xerox. Also passport office takes your photo.

 

Other Passport Annexure 

The required documents for the passport application are: Annexures A, C, D, E, F, G, H, I, and J. Additionally, you must include Appendix-12 and an Authority Letter. Please ensure all these forms are completed accurately and submitted.
