There is no annexure needed for the Tatkaal passport application.

 

How to fill “Annexure A” for an Identity Certificate

Annexure A is used for service for government, state government, or public undertaking employee passport applications with a single declaration. This annexure is to be filled up only on a computer.

I Applicant name, father’s name, and address
II Residing time and designation
III Applicant’s name and father’s name
IV Government service ID card number
V Reference no and date
VI Applicant Photo, including office stamp and signature
G Finally, Administrative Authority Signature with Name, Designation, Address, and Tel No. 

 

“Annexure C” for a Single-Parent Minor

Annexure C is used for minor passport applications with a single parent’s declaration. This annexure is to be filled up only on a computer; it is not to be used manually.  List of details as below.

A Minor Child name and Date of Birth
B Father and Mother Information, like Name, Address, Contact No, Email, Passport No.
C Passport Application vide ARN Number
D Signature/ Consent parents name
E Reasons for single parent’s
F Finally, signatures of single parents with ID numbers like PAN, Aadhaar, Voter, or Passport.

“Annexure D” for a Two-Parent Minor

Annexure D is a service for minor passport applications with a joint declaration from both parents. This annexure is to be filled up only on a computer; it is not to be used manually—list of details as below.

1 Minor Child name and Date of Birth
2 Father and Mother Information, such as Name, Address, Contact No, Email, Passport No.
3 Passport Application vide ARN Number
4 Finally, signatures of both parents with ID numbers like PAN, Aadhaar, Voter, or Passport. 
5 Date and Place

 

“Annexure F” for a Lost or Damaged

Annexure F is used for lost passport applications with a single declaration. This annexure is to be filled out only on a computer; it is not to be used manually—list of details as below.

b GD and FIR Number
c Date, Time, and Place of Passport Lost
d Passport, Visa, or Travel Country Information
e Finally, signatures

“Annexure G” for a No Objection Certificate

Annexure G (NOC) is a service for government, semi-government, or government undertaking employee passport applications with a single declaration. This annexure is to be filled up only on a computer—a list of information as below.

i Letter serial no and date
ii Applicant Photo with official stamp and signature
ii Applicant’s and father’s name
iv Employee designation and time for service
v Signature of Controlling/Administrative authority
vi Administrative authority, full details, including phone number.
vii Date and Place

 

“Annexure J for a Marriage Certificate

Annexure J is provided for the marriage certificate of passport applications with a joint declaration. This annexure is to be filled up only on a computer—a list of information as here.

1 Name of applicant with address
2 Spouse name
3 Passport-issued spouse’s name
4 Joint photo of husband and wife, including self-attestation
5 Signature of husband and wife with PAN, Voter, Aadhaar, or Passport No.

Annexures Key Points

a Mandatory: If you do not submit the necessary annexure, your application will be put on hold at RPO until it is provided.
b Plain Paper: Most annexures are signed on plain white paper, with the following instruction.
c Truthfulness: Furnishing false information in any annexure is an offense under the Passports Act, 1967, and can lead to penalties.

Q. What is Annex A in the passport application?

A. Annex A is an identity certificate. It is an issue for government employers for your employees. It is used for ID proof, address proof, and date of birth proof, for the applicant.

Q. Who can sign Annex A?

A. Annex A is an identity certificate, signed by the head of the department officer as per rank. Also need to mention Name, Designation, Address, and Tel No.

Q. Who can sign Annex G?

A. Annex G is signed by my government employer, the head of the department. it is necessary to provide the name, designation, address, and TEL number of an employer.

Q. Who can sign Annexure H?

A. Annexure H is the prior intimation letter. It is signed by the head of the employer with an office seal.

Q. Which annexure is required for the reissue of a passport?

A. No need for an annexure for reissuing a passport, but if an applicant is a government employee or minor, they need annex G and annex D and C.

Q. How do I fill out Annexure D for a minor passport?

A. Annex D: Fill up a process like the name of the parent/guardian applying for a passport, resident address, name of the child, signature/consent of the father or mother, father or mother is traveling in India or abroad, signature of a parent or guardian, the date, and place.
